Other Forms Required for SOME Glenwood Students
- Health Physical - Illinois Law requires all incoming ninth grade students to have a current physical and proof of Tdap shot before the first day of attendance.
- Sports Physical - Student-Athletes only
- Asthma / Anaphylatic Administration Permission
- Medication Authorization Form - Required for medications other than over-the-counter to be administered during the school day.
Return forms 1-4 when necessary to Office I at GHS.

- Student Parking Application - Return completed form to Office II at GHS before August 30, 2012 for students who wish to park at GHS. Should you have questions on this form please call Office II at (217) 483-2424 Ext 73.
- GHS Parent-Teacher Organization - Complete the form online.
New students entering from outside Ball-Chatham must register in the GHS Guidance Office. Should you have questions, please call Guidance at (217) 483-2424 Ext 3.
